Which year’s income sets your 2026 premium
Your 2024 return does. Social Security works from income two years back, starting with adjusted gross income on line 11 plus tax-exempt interest, and whether your Social Security is taxable feeds in.
These are cliffs, not phase-ins. One dollar over $109,000 costs $1,148.40 a year in Part B and Part D surcharges combined.
When you can ask Social Security to lower it
Only a qualifying life-changing event — retirement, divorce, a spouse’s death — lets Social Security use a more recent year, through its process for requesting a lower IRMAA. A one-time capital gain is not on the list.

Why the hold harmless rule rarely helps in 2026
The hold harmless provision caps your Part B increase at your COLA increase, so an existing check cannot fall. This year the average $56 COLA exceeded the $17.90 rise, so almost nobody is protected.

Medicare bills you directly, usually quarterly, and Medicare Easy Pay can debit your account. This catches people who delay claiming Social Security but enroll at 65 — a bill arrives, not a deduction. Once you apply, billing switches over.
